I started this service after seeing firsthand how overwhelming medications and healthcare can become. With over 25 years as a licensed clinical pharmacist, I help simplify medications and healthcare when it starts to feel overwhelming. I review medications for safety, explain conditions and how treatments work, and make sure you feel confident in your care.

A client reached out when he wanted to travel to meet his new grandchild but was the primary support for his sister at home. During his time away, I stepped in to provide weekly pill box setup, regular check-ins, and coordinated additional support with the local Council on Aging, so he could travel with peace of mind, knowing she was safe and cared for.

I was asked to visit a homebound senior managing a large and complex medication regimen. During my visit, I reviewed her medications, asked detailed questions about her health and routines, and reconciled her list with her pharmacy. Afterward, I created a personalized medication assessment for her care team, identifying duplicate therapies, potential interactions, and opportunities to simplify her regimen. Her team welcomed the insights, helping improve coordination, safety, and overall confidence in her care.

Bringing Clarity to Complex Medication Regimens

After a recent hospitalization and new diagnosis, a patient needed help understanding her medications and how they supported her health. She was feeling overwhelmed and unsure what to continue taking. During her home visit, I discovered she was still taking medications that had been discontinued, along with expired supplements. I coordinated with her provider to reconcile her medication list, clarified her regimen, and explained each condition and medication in simple terms. I also organized her medications and set up ongoing refill support. She felt confident, organized, and better equipped to stay on track and avoid another hospitalization.

A client wanted to feel more confident going into her medical appointment but wasn’t sure what to ask or how to make the most of the time. Together, we prepared for the visit by reviewing her medications, clarifying concerns, and creating a simple list of questions and priorities. We also ensured her medication list was accurate and easy to share. She went into her appointment feeling organized, confident, and better able to advocate for her health.

A client’s family was unable to attend an important doctor’s appointment due to work obligations, but wanted reassurance that their mother’s concerns would be addressed and that everyone would have a clearer understanding of her care moving forward. Before the appointment, we discussed her goals and prepared key questions. I met her at the visit, helped guide the conversation, and ensured her medications, concerns and careplan were clearly communicated. Afterward, we reviewed everything together, and I provided a clear summary for her and her family. She left the appointment feeling heard, informed, and confident in the next steps of her care.

Advocacy and Support During Medical Appointments

SoCo Rx Consultants offers free 30-minute one-on-one medication reviews for seniors through local Councils on Aging. During your visit, we will review all prescription medications, over-the-counter products, vitamins, supplements, creams, inhalers, and eye drops to check for safe dosages, possible drug interactions, and help you better understand your medications. We can also discuss the best times to take medications, whether to take them with food, and answer your medication-related questions in easy-to-understand language. Bringing the actual medication bottles and products is much more helpful than bringing only a medication list. Pre-registration with each Council on Aging below is required — please contact them directly to sign up.
